Use the CHP Provider Directory online search to find physicians within the CHP network. 

Search providers with MyCHP Service

We have a Provider Directory under MyCHP Service and when you login, you can search information about providers, who are included in the insurance product you have.

Access MyCHP Service now and enter your username and password. Your username is your Member ID number and your password is "Password1" the first time you log in. You will be prompted to change your password after your first log in (please remember your new password for future reference).

****When selecting a provider, make sure the provider you obtain services from is contracted with Community Health Plan (CHP). When a provider refers you to a facility for services, please ensure that the facility is contracted with CHP. In order for benefits to be payable at the highest level possible, you must obtain services from a CHP contracted provider at a CHP contracted facility.

Many providers practice at several locations but do not contract with us at all locations. Please log on to the CHP Web site at www.mychp.com or contact CHP’s Customer Service Department to verify provider participation at your desired location.****
