Disease Management
Community Health Plan offers Community Connections Disease Management programs to members who have been diagnosed with diabetes, heart disease, COPD or asthma. The program's focus is on member education and instruction on how to self–manage a chronic disease. The goal is to promote overall health and increase quality of life.
What you can expect from participating:

Regular contact with a registered nurse

Communication with your doctor

Individualized goal setting

Educational mailings on topics such as diet, medications and a healthy lifestyle

Quarterly newsletters
